Case Management is a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the services to meet an individual’s healthcare needs following benefit plan guidelines to provide quality and cost-effective outcomes. The process usually involves input and support from many individuals including the member, their family/friends, care providers, and other suppliers of the healthcare system. All work together to achieve agreed upon goals for the member.
Should Case Management fit your needs you are encouraged to enroll through your Primary Care Provider (PCP) or you may contact Healthcare Resources NW Case Management Team at 1-877-261-6991 from Monday through Thursday, 8:30 am to 5 pm and Friday, 7:30 am to 3:30 pm.
A member has the option to decline participation in case management at any time.
