Member’s Bill of Rights
As a Health Plan Member, you have a right to:

  1. Confidentiality and privacy of health information in accordance with state and federal law.
  2. Compassionate care that is respectful of your cultural, psychosocial, spiritual and personal values, beliefs and preferences.
  3. Be understood and treated well, even if you have limited English skills, a different cultural background, or a disability.
  4. Receive information in a language or method you can understand.
  5. Designate or authorize another party to act on your behalf.
  6. Express concerns and/or file complaints about care and services you receive without fear of reprisal.
  7. The right to refuse treatment or services.
  8. The right to know health information may be shared with your health plan and its practitioners/providers.
  9. The use of end of life and advance care directives as appropriate.

Case Management Bill of Rights
In addition, as a Case Management Member, you have a right to:

  1. Participate in the development and implementation of your Case Management plan of care.
  2. Receive a copy of your Case Management plan upon request.
  3. Understand the reasoning for being selected for Case Management and for ending or closing Case Management services.
  4. Be informed of choices regarding Case Management services.
  5. Refuse Case Management services.
  6. Access to alternate approaches if unable to fully participate in Case Management.
